MLA Citation for a poem that was printed in a handout created by my English teacher.?

I need an MLA citation for the poem "The White Man's Burden" by Rudyard Kipling, but the catch is that it was printed in a handout which was created by my English Teacher who I'll call John Smith for these purposes. Does anyone know how I would cite this?
